Paper Direct Debit sign up forms

This isn't directly RE-related but is part of the broader process I'm working on so would value hearing anyone's experiences. As we move DD signups to mainly being paperless, we are also reviewing our paperwork for oldschool paper declarations. Has anyone got examples they could share, particularly of incorporating it into a leaflet, and where the DD Guarantee text is? We're getting advice from our bank but it would be great to see 'live' examples. Thanks! Pete
