Reporting Question

Getting numbers of specific donation form from a specific page. Now to give you some history of what happened with these forms are as follows; there are three different donation option on this page: .

Now these forms had a generic source and sub source before yesterday. That was put in place in order to place those forms and get things going. I have since been asked to change them in which I did... now this is where it gets a bit tricky. I'm being asked to get the total amount of donations before the change to the codes for those three donation forms and only from that page.

Now what I'm thinking is we can track the click thru for that page itself... am I correct? I tried to find that info but I got nowhere using Google analytics, I may be doing something arong. I was a bit lost and was hoping that someone could shine some light on it to find that info out and generate a report.

How can we run a survey report and attach Member IDs to the report? For example, I want to download the Excel file for a survey, but I also want the Member ID attached to each record. How can I do this?
