Changing the PDF document for default printable gift form

Hello everyone, I need help with changing the gift form that is currently uploaded in the link for donors that which to physically send in their donations. This is a problem for a particular Team raiser. I have been to the 'customize page' section in the blueprint but the issue is beyond that. I need to know how to change it as the builders of the site made a mistake and put in the wrong form. Thanks for any insight.
