We are wanting to post a sign-up form for our employee giving that can be emailed to us once completed.  What form do I use?  I don't want to use an event form because it needs to be linked to an event.  A donation form will not do because we enter the donation amounts separately into our payroll system.  Any advice?

    Hello Karina:


    If I'm understanding correctly, you want an online form for staff that allows them to sign-up for payroll deductions / donations. I would use a NetCommunity form, and create the necessary fields (title, first name, last name, department, gift amount, frequency, etc.)  The nice thing about forms is that you can collect the data in several ways:


    1. a person or people in your org can receive an email that someone has completed the form

    2. You can download a CSV file of the data

    3. You can download each sign-up as a PDF


    This way you can pass the confirmation along to your payroll department to start the payroll deductions.

    I've gotten the form to work but now I'm having trouble figuring it out how to send an email notification when a form is submitted.  Gotta figure this one out!

    If you are using a NetCommunity Common Form, go to the Submit Settings tab in your form. At the bottom is a check box for send an email notification.
