Membership registration fee plus optional donation form

I'm hoping someone can help me with developing an eTapestry DIY form that includes both a membership fee (tax receiptable) plus a tax receiptable donation option. I can't seem to figure out how to add an additional donation option block on the membership registration and fee form. Is this even possible? Any help/advice is greatly appreciated.
