Custom Form- Insert Date Field

I am creating a custom form used by grantees to verify wire transfer receipt but I am not able to include a field where the grantee can type in the date they received the wire transfer. Has anyone else found a solution for this without having to add Blueprint?

My other option is to use file upload box so the grantee can add proof of receipt but I can see this being a challenge for some organizations.

The ability to collect wire transfer information via a custom form may resolve this issue in the future. ?‍♀️ Any feedback now would be appreciated!
