Donation form not active

I have researched and searched and can't figure out why it's not working. We created a new donation form for our current campaign and our website people can't get the java script to work. It says…"This form is currently unavailable…" How do I make it active?

Have you “allowed” the website that host the form whitelisted in Online Express?

RE database view > Online Express > Account > Integration Option > Form Security
