Blackbaud checkout mobile responsive forms not working

Hi, i've been trying to implement the blackbaud checkout forms from the downloadable kit but it doesn't look like the example included for design B (with the cat as the background) at all. I have no clue what is wrong here and wondering if anyone has also ran into this issue?
