Questions for staff to answer when requesting a query

We do not currently have a RE Admin and I am being asked to pull queries by Development staff. Does anyone have a template they have created to share that staff would complete when requesting a query/export/report? The template would ask the staff to consider the many parameters necessary to successfully give then what they are looking for
