Add on's to membership levels

We have the ability for our members to add one adult or unlimited children onto three different levels of membership that we offer. I can not figure out how to add that to our online form to allow renewals or new members to do that through online express. Any help would be appreciated.

These “add ons” have an additional feel.

$15 plus tax for an additional adult

$10 plus tax for each additional child.
