Confidentiality Agreement/consent form

Hi, I hope someone has had experience with working with parts to gather consent or confidentiality agreements from their HigherEd Alumni Association Board members. 

We limit what groups we have with logins into our NetCommunity Communities. We would like our Alumni Association Executive Board of Directors be a group that we would allow access to our directory etc. but we will need to have them sign a confidentiality agreement and have that as part of a condition of their new user registration. I take it that I will need to create a form, but what form, is there any way to manipulate the communications consent form?
