Confidentiality Agreement/consent form

Hi, I hope someone has had experience with working with parts to gather consent or confidentiality agreements from their HigherEd Alumni Association Board members. 

We limit what groups we have with logins into our NetCommunity Communities. We would like our Alumni Association Executive Board of Directors be a group that we would allow access to our directory etc. but we will need to have them sign a confidentiality agreement and have that as part of a condition of their new user registration. I take it that I will need to create a form, but what form, is there any way to manipulate the communications consent form?

All users are required to fill out a Confidentially Agreement before having access to our database. Attached is an example of one of several if that helps.

Thank you, I think we may use some of the text you provided as we create ours.

Do you have your folks sign this electronically or do you have them download it and sign and email back? I am looking for a way to "DocuSign" the agreement.

We have them download it to sign, so we can provide a copy for them to keep as a reminder.
