Issues with form formatting - field positions are not loading correctly.

About two weeks ago we started noticing forms loading with incorrect placement of some of the fields. We use Chrome as an organization, and I've seen this happen on multiple PCs. If we hit reload the formatting gets fixed and loads fine. I've attached an image so you can see exactly what is occurring.



We had to have a custom snippet of code added to our forms because of some style sheet issues. So one of your team gave me a code snip to add before the form code. I mention that in case its relevant.
