Don't know of a site.  Did you find any examples in the shared document forum? 

We keep it as simple as possible. 

Our letters to the donor thank them, verify gift, fund and use of $, that acknowledgement is being sent, tax statement if appropriate. 

Our letter to family states that a gift has been receive in memory/honor of ______ to XX fund, how that $ is used/impact gift will have, who donor is with address.  That's the basics. 

For memorial for Y member/past board member who's involvement with the Y is well known that is usually referenced as well.
