Release of Restricted Funds Form

My Foundation is supporting organization; all our funds support the <?xml:namespace prefix = st1 ns = "urn:schemas-microsoft-com:office:smarttags" />
Medical Center.  Right now we have a form to complete for departments to request restricted funds.  I'm curious if anyone else uses a form and if so would you be willingly to share it?  Also does anyone do this electronically? If so did your auditors have a problem with electronic signatures?<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" />
