Sorry, don't have anything special to share.  We use the same pledge card that we use for all prospects.  We just utilize a tear-off stub that normally has giving history for them to enter payment info - check boxes for payroll deductions vs cash.  They complete the same HR form for the payroll deductions as they do for other payroll deductions.
