add a question to an online form

Hi there;


On most of our paper response forms, we have two tick boxes - one with: "I have remembered the Foundation with a gift in my Will" and one with: "Please send me more information about giving to the Foundation in my Will".


Not wanting to clutter up the form, but might it be possible to add the ability to ask a question or two at the end of the form?

    We did this same thing. I added it to the form using a gift attribute. Then it is attached with the gift that they submitted to you online. Then query on that code on a regular basis. We do monthly.

    Thanks Steph!  This could work - but several challenges:


    I created two attributes as yes/no, but OLX seems to want these to be required fields.  I don't want to force people to fill this out.  I don't want "no" as the default, either.


    OLX places these right after the gift amount - I need these questions to be at the end - just before the submit button, I think.  Can't see a way to rearrange..........


    The box for the yes/no dropdown is really wide and aligned with other fields in the form - this means my question wraps to 3 or 4 lines.  I believe the box could be set to align with the right side of those other fields - that'd give more room for my questions.
