OLX donor form

I too needed to add 2 questions to a donation form, and so I used the attribute option, you first need to add the attribute that you want on the form to the attribute table, then can change select the attribute(s) in the OLX form, then you can change the wording as you can with most field heads. I added an attribute that was a question "Send me more information about the XX event" with a yes/no option that is mandatory to answer on the form. You can also add an attribute that is a table/text fill if you want to offer the donor a box to add text into. It then is imported with the gift when you batch it, to the attribute tab. It is also on the receipt that is sent to you alerting you of the gift. Hope that helps!
