When save a form: error sorry something unexpected happened

We have a bus register form, and one parent can't submit the form, every time when she try to submit it will give her the error : sorry something unexpected happened

    I hope this message finds you well!



    On first look, it appears that there are some characters in the dropdown which may be interpreted incorrectly by the system. Theses include use of '/' ''. These have the potential to be picked up by some browsers as design elements.



    Also, in the last screen shot, I see where you're soliciting Name and Grade in the same field and calling for a '/'. This too may be seen as a design operator instead of a value.



    As a first resolution suggestion, I would recommend eliminating the special characters from your dropdown. '#' is ok. I would also cease collecting name and grade in the same fields and give 'grade' it's own field. This will allow you to eliminate the drop-down.

    I removed all the special characters, and still get the same error message, now I try don't add the other kid, but still the same error, please check the attachment.
