Donation form options

We're trying to develop a donation form that 1) allows for special occasion giving (i.e. wedding, bar mitzvah, etc.) and 2) allows the donor to give in honor or in memory of a person, and 3) enables the donor to give to a specific fund (i.e. lung cancer, breast cancer, etc.)  What is the best way to approach this when building the form?

    When creating a form you can add program options these can be changed at anytime and you can have as many as you want which the donor can choose from.  You can also add frequency options - sustaining donor options. You can see one of our forms as an example at www.stjo.org/donate

    All these options are found in the data elements when designing a donor screen. I hope this helps.
