Does anyone have experience embedding a Convio survey in a Facebook fan page?

I'd like to do something similar to what MailChimp can do:

where I add a newsletter sign-up form to our Facebook fan page. We have our newsletter signup as a survey.

Has anyone tried this?

I am trying this and while I've been able to get the form to display using the FBML box, I cannot get the form to work. An error keeps occurring for some reason.

We'd love to hear from someone who's had success.
