Does anyone use a request form for OLX donation pages?

Hello,  We have several departments all utilizing OLX for their forms.  A few of us create these forms for them, but the back and forth can get exhausting.

For our regular RE reports, we have a report request form they fill out which is helpful. 


Does anyone use a request form for setting up an OLX donation form, and would they will be willing to share it?


It would be so much easier if the departments would fill out their funds, appeals, whether they want givalanche, who the email should go to...well you get the point.

    Nothing formal, but 95% of our forms are ones that I work closely with, so it hasn't been an issue.  The few we've had otherwise were a discussion via email.  It wouldn't be too difficult to set up a quick form, though, as it's pretty straight forward.  I would go tab by tab and ask what they want.
