Wanted: Data Maitenance Request Form

We are hoping to institute procedures around submitting data maintenance/hygiene requests. Does anyone have a form that may be of assistance?

The purpose of this form will be to replace informal/incomplete requests via e-mail for issues such as gift miscodings, constituent record errors, etc.

We have set up an excel document of fields necessary to import new names in RE. This way we import the information in RE and use the Source Code to keeep track of how they ended up in RE. As far as updating records we just e-mail the data entry person.

an online document or email template for our staff to use so when
they give us new or updated information they give us all we need to
set up the record and as much info. as possible. I don’t know
about your staff but often our staff know more information but
forget to tell us so it can be captured in the record. Such
things as primary contact changes, phone numbers and email
addresses. I was hoping someone had created a great way to
capture these rather than reinventing the wheel.

My 2 cents...

If I were asked to create said form, I would just take all the fields in my constituent batch template and put on the form. It's basically all the fields/info I want as data entry person. Fields for both individuals and orgs. If it's to handle updates/changes, I'd have boxes like this at top of page: __ Update to existing record ___ New record
