Contact Us survey form

Options
Hi, is anyone using a LO survey form on their website for something like a Contact Us form?  We have this form that is used for multiple purposes, such as students asking questions about their application and constituents asking to be removed from our mailing list.  We don't enter students in RE and we keep a separate list outside of RE for 'do not contact' people that are not already in RE.  Seems like we shouldn't be using LO for a form like this since it creates a record in LO for these people, and also pulls them over to the RE plugin.  We can reject them in the plugin but we would need to check their record in LO first.  Also, if I understand correctly, if their LO record is changed or updated it will pull into the RE plugin again.


Our website runs on WordPress and I'd rather use a contact form on that instead of an LO survey form, but wanted to see how others are using it and handling the records.

    I can relate to your issue. Although we don't use RE, we sometimes collect contact info for different reasons that are not joining Forté as a member. One of the quickest solutions I use (especially for attachments, which doesn't apply here but keep it in mind!) is JotForm.


    The form is easy to set up and drops into LO's CMS easily, so WordPress would be the same. You can choose on JotForm to receive an email notification, and if it's easier it even has a connection to DropBox if you want the form submission saved as a PDF.

    JotForm has a free option that would probably work for you, but if you have a high volume the pricing is more than reasonable.


    I don't have a referral link or anything, I just love the flexibility and solution that JotForm provides! I hope this helps.

    Hi Josh,


    We've been using LO for about 8 months now and launched our new website at the same time.  We use a LO form for individuals to "Stay Connected" but do NOT use a Contact Form from LO.  We simply have an individual email us
    Since the Stay Connected form is hard coded, it sounds like it would not be good for you to use.  You are correct that any LO form on your site links directly to Constituent 360 in LO and creates a profile which is then sent to the Plug-In to be approved or rejected by you.  Likewise, any new constituent entered in RE flows through to LO and creates a new constituent.  So, I'm confirming your thinking about not using a LO form for your students or anyone else you do not want entered in RE.
