"Authorization Date" field (secretly) required when using Permission to Treat in order to process reenrollment forms in EE

Options
In NetCommunity>Forms there is a nasty little field called Authorization Date listed directly after Permission to Treat under Student>Medical. Rather unfortunately, if the Permission to Treat field is used but the Authorization Date field is not, the NetCommunity utility in Education Edge will not process the form. There are also zero ways around this aside from asking the user to resubmit the reenrollment form, which for most organizations (particularly one with a high volume of submitted forms) this just isn't an option.

I would like to request some sort of fix to this, whether it be a popup notification when you use the Permission to Treat field that the authorization date field is also required if one plans to process the form in the plugin, or just not have EE require it at all. This is such an impractical, and apparently hidden requirement. There is no notification, warning etc. that not using this field will mean all the data in the form is rendered completely useless. On my end, the lack of that authorization date field resulted in over 30 reenrollment forms submitted in my NetCommunity plugin before I realized I had no way to process those forms.

I'm sure I'm not the only person this has happened to, since there is a knowledgebase artical about this issue that says "the form data will need to be manually entered into The Education Edge" https://kb.blackbaud.com/articles/Article/57305. Please do something about this so that the next poor soul does not have to manually enter reenrollment forms.

After playing around with this extensively myself, it appears that if you go into the medical record and check "permission to treat" and provide an authorization date, then go back into the plug in, you can in fact import the form (YAY). It would be great if both the EE and NetCommunity chat support representatives were aware of this so that the next time it happens to someone they can provide this fix. Also, it appears the knowledgebase article needs to be updated.
