"Created by" field on donation form

Is there an existing data element I can add to a donation form to allow a staff person entering a donor's information to type in his/her own name? So you would be able to run a report and see which of, say, three employees entered the information for which donation?

Donation forms can have custom fields, which you can report on in Report Writer. They can be open text, drop-downs, etc.

Under the "Select data elements to include in this form" section you should see a NEW and EDIT button. These are for those custom field elements.

I don't think so, unless someone already made a custom field for this. You can find those in a group called 'custom' in the "Select data elements to include in this form" picker.
