Responsive Donation Forms

Has anyone worked with any “plug-ins” or services that offer a great Donation Form? I was talking to a company yesterday that offers a donation form integration into RE that will allow the form to pre-fill with donor information as well as some other interesting options about monthly giving and responsive arrays based on their prior giving history. However, there are a few challenges with them that make me think they aren't a fit for us. But perhaps someone has an alternate?

Luminate Online has these options available. But it's not cheap and requires at least basic html knowledge.
