Creating Donation Forms in Luminate with Raiser's Edge integration

Does anyone have a written procedure, they would be willing to share, for creating Donation Forms with multiple designations, or a Donation Form for Honor/Memorial gifts and/or a Sustainer Donation Form?  We just moved to Luminate last week and would appreciate any insight concerning setting up these Donation Forms. For setting up a donation form, BlackBaud has some starting instructions for basic setup at:

For displaying your donation form responsively, BlackBaud provides a kit that covers some of this:

Alternatively if you want something with less coding and are using a WordPress environment for your main site, you can use an integration to handle most of the work here and embed LO donation forms automatically on your site (no coding/uploading anything etc).  Here's a couple examples in action:
