Gift Adjustment Form

Options
Hello! This forum is always so helpful! Can anyone share the Gift Adjustment Form used? Our CEO wants us to use a Gift Adjustment Form to document any adjustments made in RE/RENXT, but I am having a very hard time finding a sample or what information might go on such a form. Thanks for any help!

So, you want a form for someone to submit to data entry person that will provide info needed for the adjustment?  I haven't seen a from for that purpose but it's not hard to figure out what info you'll need:
    • what record (name and/or ID)
    • what gift: fund/campaign/appeal, date, gift type, amount
I would take a screen shot/snip of the adjustment screen tabs.  Those are the additional fields/info you will need on your form to make the adjustment.

If you're looking for documentation of adjustment made you should be able to create report/export of that info.
  • There is also the Adjusted Gift Report (in Financial Reports) which we use to document an adjustment - I provide to Finance as well as keep a copy w/the gift batch that has been adjusted. It's not a form per se, but provides the documentation you would need.
  • Attached is the form we use. Let me know if this is helpful.

We have a similar form to alert our business office as we are not integrated on Blackbaud products. I send them the canned reports each month but find when they actually have to transfer money from one fund to another it is better to alert them right away. We have created a Google form with the same questions as the one that Mary Maiden Mueller‍ shared yesterday. The thing we like about the Google form is that both the business office and I get alerts when our data entry person needs a change made. I do the majority of the adjustments in RE after being alerted by the form that a change is necessary.
