Help formatting an Appeal on Online Express where with each vote they will have to pay $20.00 with a maximum of 3 votes

Hi BB world/users,

We are running an Alumnae March Madness Appeal, and we are asking our Alumnae to vote on their favorite group, club or team they were affiliated with while here at our school. We would like to build the form so that they can vote up to 3 times, and that with each vote they would be charged a minimum of $20.00. Ideally we would hope that they could pay only once per round, paying of all of their votes.  We also do not want to have 3 votes for the same group, club, or team from one person per round.  Is there anyone who has built anything close to this that would like to share how the form was built and any hindsight issues that could be avoided?
