Phonathon Forms

We are in the process of revamping our phonathon.  We are a manual shop and have always used the canned form to print phonathon forms.  I am looking to change this as we do not get close to the information we would like on the form from the canned form (i.e. multiple phone numbers and email addresses, etc.)  Does anyone have any forms that they would be willing to share to help get me started?  Thanks.

    Here's the one we use. Not sure if it's any help for what you're looking for. I just export whatever data I need, review and make any needed corrections in Excel and then mail merge with the form in Word.

    I agree with Stephanie.  Build a template and export the fields you want to see, and then merge the export into the template.  Once it's set up, it's easy to use over and over.

    The canned form does not have nearly enough info or choices of info to make it useful.
