What are the main differences between groups and interests? Is it possible for people to self-select which groups to which they belong, sort of how they can select their interest categories?

You've actually hit on the fundamental difference between the two. The original design intent was that interests should be selected by the user and groups are assigned by the organization. There have been features added over the years that blur that distinction, but that was the original intent.

In general, the group membership logic is designed to handle larger volumes of data with better performance. For example, now if you include a group membership conditional in an email message, the system will handle that far more efficiently for a large mailing than it would an interest-based conditional.
