OLX Form for Free Event

Has anyone used OLX to track registrations for a free event? We host a couple awareness events during the year and would like to offer the possibility to register online. For example, we have a Hike and have many families attend. We need all names of the members of their family, but we don't want them to have to complete every address box for each registrant. Is there a way to have a "head of household" entry and then just names for the rest of the registrants? We also do a giveaway event and would like to have the parent name and then the names/grades of the children. Any advice would be appreciated!
