Add general text box?

Does anyone know of a way to add a general text box to an OLX donation form? For example, an organization similar to ours has the bow shown below on their donation form that lets the donor enter a statement telling why they chose to give to the organization (they use Auth.net). This is a great opportunity to collect quotes for marketing/social media purposes, and helps further hone in on your donor's "why", which can be used for stewardship and cultivation purposes down the road. Any idea on how to put this into an OLX form? Thanks!

Hi Sydney,  

I stole this idea (loved it) and used the comments box on the OLX donation form.  First you need to allow comments on tab 2 of your donation form setup.  Then on the fine tuning tab I changed the display text to read "Why do you choose to support the Annual Catholic Appeal?"  The information is collected as a gift attribute, and create a query to review.

Not sure if this helps you, but it surely helped me.

If you are already using the Comments box for something else, you could create a gift attribute which would allow for them to type in their reason for donating (I think the description fields allows for 50 characters), or you could set up a drop down menu with varied selections which would make it easier to query on in the future.That's my two cents!
