How to use "one" donation form on your site, allow all affiliates to link to it, and still be able to track gifts to specific affiliates?

Hello all, I have a few questions on how to implement this. I noticed on the Alzheimer's Association website you are able to do exactly what I want. Anyone know exactly what it is I have to do? I am assuming that you a add a custom field to the donation form like so: "&s_affid=". Then append the id# to the donation form link? Example: https://secure2.convio.net/adarda/site/SPageServer?pagename=Donation_Form_Launch_Page&s_affid=59. So, affid=59 in this case, showing that the chapter ID is 59. Is this really all I have to do? Also will this custom data be added on in reporting? We have over 130 affiliate sites and would like to have just one donation form. Please if anyone can be of assistance I would appreciate it
