What are your favorite fields to include in a failed monthly payment report?

I'm setting a variety of Repor Writers on failed monthly gifts. There are a lot of fields to choose so I'm curious who has come up with a winning combination that allows you to track your monthly failed payments (so you can reach out quick to the donor to right the problem).
