Data/Query Request Form

I am looking for samples of data/query request forms some of my fellow RE users have.  I am hoping to have a form my dept. will fill out before I perform a query.  Many times after exporting and cleaning up the list I get a request for additional information.  More time more effort!  I think a form would lead the requestee to think about what he or she really wants in a query/export and I would also have a solid outline to follow.  Also would like to track how many queries I am doing for the dept.  and this would be a good method to do so. 


Any suggestions or comments also welcomed but would love to have some samples to use as a starting point!

    I had the same issue in my office, so I created a form using Wufoo--it's a free online form builder that's pretty easy to use. I set it up to come directly to my email once submitted.

    The biggest thing has been the culture change to actually use the form. A lot of the time I will have a discussion with a team member about the requirements and then end up submitting the form on their behalf using their email--they get a confirmation email as well so they can see I have it in my list.  I have been finding that it also makes it easy for me to plan my work/projects because I can plug those emails right into my calendar so I never have to go searching for a rogue email or a scribbled note for criteria--it's all in one place! You can also log in and see all the previously submitted requests which will make it easier to duplicate criteria from previous lists.

  • Thank you so much!  I did end up finding some forms on Blackbaud after using a Google search so I was just working on a word doc but I will def. take a look at Wufoo!
