Embedding survey questions with drop downs into donation form?

Does anyone know how to embed survey questions into a donation form in order to capture information such as t shirt size for sponsors? We don't use Team Raiser because our events are sponsor driven instead of participant/team driven so I am wondering if anyone has any experience with this????

Yes, you can do this by adding Custom Questions using Donations (not Donations Classic). In Step 3 - Design Donor Screens, edit the donation form, and on the Form Designer, scroll all the way to the bottom to create a Custom Question. You can then choose to either update the member's record with the response, or not to update the member record.
