Donation forms - honor/tribute - anyone willing/able to share theirs?

I'm struggling with the creation and reporting for two new forms - one for memberships (if a membership is purchased as a gift for someone else) and one for memorials/honorariums (when an honoree or tributee needs to be notified of the donation).  I've got rough drafts of the two forms and Report Writer reports, but haven't worked out the bugs. We intend to make those gift notifications manually via snail-mail, and just need a report that will give us the information that was input by the donor.

1. Is anyone willing to share a form and a report of theirs that works?  (Why reinvent the wheel?)

2. If so, is there a way for us to get a working copy of the donation form and report that we can then modify for our use?

You might try contacting some of the folks who posted to this thread on honor/tribute giving if you don't get a response from the community in general.

Gift memberships are a bit trickier.  We have many clients who would like to be able to fully manage gift memberships from within Convio's tools, but there's nothing in Convio specifically designed to manage start/end dates of gift memberships and the relationship between the giver and the recipient.  It gets very complicated very quickly.

What I can imagine, however, is setting up a donation form specifically for gift memberships in Convio and using the honor/tribute fields to allow the donor to designate the recipient.  You can re-label those fields on your donation form so that they make sense in context to your donors.  Once you're up to speed on using Convio's eCard tools, you can even allow the donor to send a branded notification to the recipient of the gift membership.  All of the membership management, expiration date (if applicable) and relationships between donors and recipients will have to be maintained offline, however, in your database of record.

I don't have specific gift membership examples I can point you to, but will do some digging.  We don't have any Go! clients who have set up gift memberships.  If I'm able to find examples from other Convio clients, I'll post them back here.
