Research registry solution?

We're considering creating a research registry and are in search of some options.  A simple solution is to create a sophisticated secure survey using the Convio tools then taking the info offline to sort and match with a research project.  I don't know what our anticipated volume will be (nor the # of questions in the survey) so I'm not sure this solution will work.  I believe some OTS (off the shelf) registry databases exist and I'm wondering if other health organizations might have any insights or suggestions for how I could/should move forward.
