How can my constituents log an offline interaction with a decision maker?

Create a survey.

In step 1 of the survey, "Identify Survey", do the following:

    • Rename the "Submit Survey Button Label" to something more relevant, e.g. "Submit an Interaction".
    • Check "Yes, allow the same person to submit this survey more than one time".
    • Set the "Survey Submitted Page" to a relevant page, e.g. the "ConsProfileUser" page.  Using the "ConsProfileUser" page will allow the constituent to update their profile after the fact.  Also, if you have a link to this survey on your ConsProfileUser page, this will take them back to that page after submitting an interaction.

In step 2 of the survey, "Include Questions", include relevant questions, e.g.:

    • Question type "Short Text Value (40 characters)": "Decision Maker Name:".
    • Question type "Multiple Choice Single Response (Drop Down)": "Select the type of relationship you have with the decision maker:"
    • Question type "Multiple Choice Single Response (Drop Down)": "How well do you know the decision maker?"
    • Question type "Multiple Choice Single Response (Drop Down)": "What type of interaction have you had with the decision maker?"
    • Question type "Date Value": "When did the interaction take place?"
    • Question type "Text Value": "Details of the interaction:".

In step 3 of the survey, "Configure Autoresponder", select "Do Not Send this Autoresponder".

In step 4 of the survey, publish the survey.

Embed the survey in a PageBuilder page.

  • Create a single component, reusable PageBuilder page.
  • Select the "HTML Content" component type.
  • Select the gear to a "Convio Component".
  • Select "Survey", "Survey" again, and then your newly created survey.
  • Publish the PageBuilder page.
