What if donor/billing addresses are NOT the same?

We recently upgraded our security to require that the billing address match the address on the credit card. We've had a few donors run into a problem when they're making a personal gift using a business credit card. When they put in their home address (where they want us to contact them) using the organization card, the transaction fails unless the addresses are the same.

We would like to have an additional address field drop down when a "billing and donor addresses are not same" box is unchecked (it would be set to checked by default). Something like the honor fields drop down---you wouldn't see the fields unless you unchecked the box.

I haven't been able to figure this out without showing the address prepopulated in both sets of fields, with both sets of fields cluttering up the form view.  None of the form elements listed below seem to do the trick. Any suggestions on how to get the "not same" option to show only if donor address and billing address are not the same?
