Logo on a form?

Can you add a logo(s) to an individual donation form on OLX? For example, I am creating a form for our golf event and I would like to add our golf logo to it.

You cannot add a logo to a Donation Page.  However, the Donation Page resides on a page of your website, so you can have the logo in the header of that page.  See this question and answer from BB staff.
