Associating an image with a constituent

We're putting together a survey where the results for each individual will be public. With each individual's results, we'd like for them to be able to upload an avatar. I'm pretty sure there is no way to upload an image in a survey, is that correct? Is there any other way we can do this? Is there a way to associate an image with a constitutent record? We were dabbling in using StoryBuilder, but I think that requires the user to be logged in, and we don't want to do that.

Any thoughts?
