Examples of successful single sustaining giving and single gift forms (all-in-one form)?

i'm wondering if people have tested or are using an all-in-one form that allows donors to make a single gift or a sustaining gift. i've seen a few othese, but i mostly see forms that link to each other (i.e., single gift has link to sustaining form, sustaining form has link to single gift form) and forms that toggle between, depending on clicking single gift or sustaining gift. i'm wondering about the success (i'll take failures as  well ) stories of a form that offers both options and performs better than separate forms. i know that these forms would need to be created outside of the standard convio tools.

    A standard donations 2.0 form would actually be capable of handling both one-time and recurring gifts -- so, logistically this wouldn't require APIs to test.  There are definitely pros and cons to the all in one vs. separate forms.  Most notably, I think, is the gift string.  The gift string you may want to suggest for recurring gifts would be a lot smaller than what you'd suggest for one-time gifts.  Additionally, if your campaign is specific to recurring giving, the call to action at the top of the form may focus on the value of a monthly gift and how that may be of greater support to the organization.  The best advice I can give to you would be to test this for yourself.  What works for one org doesn't always work for others.....
