5k Run- how can i add the disclaimer to the form?

We are having a 5K race and are doing online registrations.  We need to be able to add the runner disclaimer (per legal).  Is there a way to do this when folks register?  Also, can we have them add a shirt size?  This form is good for initial registrations but it creates a lot more follow-up work for us.  Suggestions?

Hi Cheri,

You could use the participant attributes (on the field options tab) on your form to capture shirt size and ask if they have read the disclamer. Perhaps you could add a link the disclaimer on the web page you add your online express event form.
