Style Guide for Addresees/Salutation?

I have a 3 page guide I printed off somewhere in forums but it doesn't have any source listed and I unfortunately didn't mark where I got it.  Maybe from one ot the sources referenced in the above thread. 

Did you try a search online for it?  Old school method - I used to use a reference page in the back of my big old dictionary sitting on the shelf. LOL
