Online Membership Form to Share

I'm wondering if any orgs out there that have a membership base would be willing to share their online membership forms?

 

We currently have ours set up using a donation form. However, it seems to be confusing a lot of our new website visitors. Is there a way to customize the areas on the donation form that say "gift" or a place to add additional text through an html box?
