Basic Flexible Sustainer Donation form using jQuery verification/UI

This is an example of a basic a custom Donation API implementation that is backed by a Flexible Sustainer form template. It is intended as a as a test bed for documentation and also as a starting place and example for Donations API customization. There are several parts of the form that must be changed to work with the form, marked by TODO comments: (1) The form action=URL must be directed at the target Convio site. (2) A shadow Donation form must be created using the Flexible Sustainer template. (3) The API_KEY parameter must be updated to reflect the API Key of the target site. (4) The form_id and level_id codes (and the associated $ amounts) must be changed to reflect the IDs associated with the shadow donation form. (5) Links to jQuery libraries and CSS styles must be updated appropriately.
