Application Questions - Share

Options

Is anyone willing to share their general application and post-acceptance questions? We were able to map new data import fields to eliminate questions on our Gen app, but we're still unable to capture everything through the import. Sample questions would be greatly appreciated.

We also have scholarships that support internships, study abroad, research etc. We need to craft more engaging post-acceptance questions to better gauge impact.

    I'll attach ours here. I believe there is another post with a similar question about general application questions. You may be able to get more answers looking there too!

    Our Post Acceptance is very simple. We require a thank you letter and a photo that can be shared with the donor. We recently added an ‘information release" which states: “By submitting this form, I declare that the information provided is true and complete to the best of my knowledge and that I have completed this form voluntarily. I give permission to disclose this information to the donor or representative of the named scholarship for which I have been awarded and for other donor stewardship initiatives.” Students may opt to agree, disagree, or choose a third option which says “I have extenuating circumstances that require my name and photo to be kept confidential. I would like to be contacted by a Donor Relations Representative to discuss my options.”

  • @Klysicia Young - At WesternU, we have a very decentralized awards process, so our general application is just two checkboxes… Then there are various conditional and opportunity applications that follow.:

    1) “I have reviewed the Applicant Record (bottom left of screen), which has been populated from my official student record, and confirm the information is accurate to the best of my knowledge. (If you need to update or change any information, please contact the Registrar’s Office at…”

    2) “I hereby authorize the release of 1) information contained in my student records and scholarship application(s) to the appropriate scholarship selection committee(s) for the purpose of evaluation and scholarship award selection, and 2) my name, college, program, year, and application/questionnaire responses, but no personal contact information, to the donors of any scholarships I am awarded.”

    As for post-acceptance, I'm attaching the end result of our post-acceptance questionnaire we used last year (Scholarship Donor Thank You Packet - Sample.pdf). You'll see we have a thank you note and photo, as well as short and long answers to create a scholarship recipient profile. These were very well received by our donors. For this year, we're still collecting roughly the same information, but we found that some of the longer profile questions overlapped with what students tended to write in their thank you notes, so we consolidated things a bit. We also ask students for their consent: “Do you consent to the University using information from your application and this questionnaire, such as your name, program, class year, photo, and quotes from your essay(s) and thank you note, but no personal contact information, for online and print promotional purposes?” Almost all say yes.
