Giving form stopped working

A donor called us to tell us our giving page wasn't working. She was right, you couldn't even select an amount and the contact info had a bunch of extra fields.



deleting the form code and pasting it in fresh fixed the problem, but given that we had not edited anything on the page on which the form resides in recent memory. I really want to know why this happened and what is to prevent it from happening again.
